Community Health Improvement Plan
The coalition's current work is driven by the 2024-2027 Community Health Improvement Plan (CHIP). This plan is created every 3 years in response to the Community Health Assessment (CHA) completed by Beyond Health. The goal of the CHIP is to identify key health priorities for the community to improve overall health and wellbeing.
In collaboration with community organizations and members, the Coalition for Change is working to improve health outcomes in Focus Area 1: Mental Health and Substance Use. Specifically, preventing excessive alcohol use and increasing resources to prevent opioid-related overdoses.
Youth Prevention

Through conversations with community members, the coalition has identified youth prevention as a priority area. As a first step, the coalition is focusing on building relationships with schools and youth-serving organizations in Brown County, Wisconsin. This has included meeting with school staff, conducting youth focus groups, and mapping community resources related to substance use and mental health.
The goal of this work is to decrease current use and prevent future use of alcohol and tobacco products (including vaping) by youth. For more information on the latest Youth Risk Behavior Survey (YRBS) click here.
